Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary
You’ll use your clinical skills to complete assessments, develop personalized care plans, and remove barriers by connecting patients to essential resources. This is a meaningful opportunity to make a profound impact while working with a diverse population in a supportive, multidisciplinary environment.
Essential Functions and Responsibilities
- Provide supportive services to individuals diagnosed with HIV/AIDS and their significant others.
- Help patients access medical, psychosocial, social, financial, and related services throughout their care journey.
- Conduct comprehensive assessments and develop individualized care plans tailored to patient needs.
- Connect patients with appropriate community resources and continuously monitor and adapt care plans as needed.
- Collaborate with internal and external partners who serve individuals living with HIV/AIDS, ensuring equitable support for people of all backgrounds and identities.
- Complete psychosocial screenings and medical history reviews for new HIV patients receiving care through Eskenazi Health Infectious Disease.
- Facilitate completion of care coordination and release‑of‑information agreements.
- Participate in multidisciplinary staffing on clinic days and provide timely documentation for encounters, recommendations, and referrals.
- Assist patients with accessing emergency financial assistance (DEFA) and other financial support programs.
- Connect patients to basic‑needs resources, including food, clothing, and shelter.
- Build strong relationships with community agencies that support individuals affected by HIV.
- Provide counseling and support around end‑of‑life issues when appropriate.
- Attend required trainings supported by federal, state, and county grant funders.
- Support grant writing efforts related to the care coordination program.
- Provide assessments and resources to HIV‑positive patients admitted to the hospital and coordinate with inpatient social work teams for discharge planning.
- Adapt services to meet the unique developmental and cultural needs of patients across the lifespan.
Job Requirements
- Master's degree in social work (may be obtained after hire if already enrolled per the department's discretion)
- Current LSW or LCSW
- Social work experience in a health care setting is preferred
